```Duties``` - Conduct patient assessments to determine their needs and develop individualized care plans - Collaborate with healthcare professionals to coordinate and facilitate patient care - Monitor and evaluate patient progress, making adjustments to care plans as necessary - Provide patient education on disease management, treatment options, and preventive measures - Assist patients in navigating the healthcare system and accessing appropriate resources - Advocate for patients' rights and ensure they receive quality care - Document all interactions and interventions in patient records ```Qualifications``` - Bachelor's degree in nursing or related field (required) - Current state licensure as a Registered Nurse (RN) or Social Worker (SW) - Proven experience as a case manager in a healthcare setting - Strong knowledge of patient care principles and practices - Ability to perform diagnostic evaluations and interpret medical information - Excellent communication and interpersonal skills - Familiarity with electronic medical records systems - Ability to work effectively in a fast-paced environment Skills: - Patient care - Inhale - Diagnostic evaluation - Patient service - Patient assessment - ER experience - Primary care experience - Hospital experience (ICU, pediatrics) Please note that this job description is not exhaustive and may be subject to change based on the needs of the organization.
